During the inspection, several outfalls were noted that need maintenance: Additional rip rap is needed around and under outfalls 2, 3, and 10. Rip rap is needed along the left side of outfall 4 when facing downstream. The soil above this outfall needs to be stabilized. Rip rap needs to be placed around outfall 6 with a rock check dam installed at the base. Rip rap also needs to be placed over the exposed geotextile fabric at the base of outfall 7. Furthermore, there were several areas of concern which could produce sediment laden runoff and cause an illicit discharge. Specifically, the exposed areas upstream near outfall 6. These areas need to be stabilized. It is also recommended that additional measures be taken to ensure that an illicit discharge does not occur. Please address the above issues within 60 days of receiving this report: a follow-up inspection will need to be conducted. If the above issues are fixed before the 60 day period is up, please contact us for an inspection. There were also multiple Discharge Monitoring reports, as well as Qualitative Monitoring reports, missing from the current permit cycle. Ensure that a copy of each monitoring report is kept on file at a minimum of 5 years prior to the current monitoring report. However, NCG12 there is currently a requirement for landfills to have and operate from an approved erosion control plan. This plan was on site and appeared to be implemented. Qualitative Monitoring Has the facility conducted its Qualitative Monitoring semi-annually? ❑■❑❑ Comment: Qualitatiave Monitoring reports were missing from both monitoring periods during 2015-2018, as well as the second period of 2020. Analytical Monitoring Yes No NA NE Has the facility conducted its Analytical monitoring? ❑ 0 ❑ ❑ # Has the facility conducted its Analytical monitoring from Vehicle Maintenance areas? ❑■❑❑ Comment: Semi-annual Discharge Monitorinq Reports were missinq from the 1 st monitorinq period of 2015, 2nd of 2016, 2nd of 2017, and 2nd of 2020. However, a summary of data was given that contained data from some of the missing reports. Outfalls 4 and 5 triggered monthly monitoring requirements in December of 2015, but it appears that only semi-annual monitoring was conducted on these outfalls after February 2016. Outfalls 2 and 6 also triggered monthly monitoring requirements after February 2016, and it also appears that only semi-annual monitoring was conducted for outfall 2 after December 2018. Tier releif letters could not be found releivinq the permittee of monthly monitoring requirements for outfalls 2, 4, 5, and 6.
