Attached is the overdue invoice for the Stormwater Annual Permit Fee for the GROWMARK, Inc. Lumberton, NC
Fertilizer Facility. In October 2020, GROWMARK, Inc. purchased this facility from Southern States Cooperative, Inc.
(SSC). The invoice is incorrectly addressed to Lauren Faulkner in Richmond, VA (Ms. Faulkner does not work for
GROWMARK) at the previous owner/operator (SSC).
Please revise this invoice and send it to me. I will process it for payment.

Attached is a copy of the Overdue Annual Permit Invoice for the Lumberton Fertilizer Plant (NCS000541). To update
billing contact information, please send an e-mail to Aziza Selkane, [e-mail], including the name of the
billing contact person along with the appropriate mailing address for future invoices.
Regarding the appropriate Stormwater DMR Form for analytical monitoring, continue to use the Generic Individual
Permit DMR Form, which is attached. I have also attached the Qualitative Monitoring Form for recording the visual
monitoring.

Left a voice mail message for you, while exploring the DEQ's General Industrial (stormwater) Permits webpage, I was
able to navigate to the attached permit summary report.
I noticed in the Permit Annual Fee section that the billing month was April and the annual fee of $860 is overdue.
This would be the first invoice now that the Lumberton, NC Fertilizer Facility has been owned/operated by GROWMARK,
Inc.
Since March 2020, 1 have been working from home. I do not recall seeing an email from the DEQ with this invoice or any
correspondence in the US Mail at the GROWMARK, Inc. Bloomington Office (I regularly check my inbox).
Can you look into this or provide me with the proper contact person within the DEQto resolve this annual fee billing?
