Since our initial registration launch, we have updated the form to include an Electronic Signature Agreement section
have linked that portion of the website above.
In order to register you and John Bird, please print pages 4-7 and each of you sign a page 7.

Please see attached for completed required eDMR registration forms. There are several updates that are required on
the Owner Verification Report for the facility:
• Please remove Lynn Farley as the ORC Backup noted under permit NCO040711 and replace with Patrick
Mays, Certification #: PC-2/1009460, Cert Status: Active, ORC Designation: Primary
Also, please update the following information per the Stormwater Permit Summary report guidelines (found here):
• Facility Contact Person: Please remove Yvonne Couts and replace with Savannah Carroll — all contact
info is the same except the email is [e-mail]
• Permit Contact Person: Please remove Jim Skinner and replace with Savannah Carroll — all contact info is
the same as above
