Subject: Request for Additional Information
Stormwater Permit No. SWA000079
United Rentals — Salisbury
Rowan County
Dear Mr. Simmons:
The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater
Management Permit Application for the subject project on September 30, 2020. A review of that
information has determined that the application is not complete. The following information is
needed to continue the stormwater review:
1. Documentation from the NC Secretary of State or other official documentation showing
the titles and positions held by the persons signing the application is required [15A
NCAC 02H . 1043 (3)(b)(ii)]. Please provide official documentation showing that Mr.
Kenneth J. Perkins has the authority to sign on behalf of the corporation.
2. Please provide a copy of the lease between 914 Properties, LLC and United Rentals, Inc.
[15A NCAC 02H .1043(3)(c)].
3. Please provide a copy of the property deed to prove ownership by 914 Properties, LLC.
4. Dry detention ponds are not primary SCMs and therefore do not meet the runoff
treatment standard. All runoff from proposed BUA must be treated in a primary SCM
before being discharged. Please note that fast track submittals do not entail a full
technical review and SCM designs will not be reviewed prior to issuance of the
Authorization to Construct Permit. The design professional is fully responsible for
ensuring that any SCMs meet the Minimum Design Criteria. A full technical review will
be conducted upon receipt of the as -built plans. Any deficiencies found in the as -built
plans must be corrected before issuance of the final permit at the expense of the applicant
or the Department will initiate compliance and enforcement [ 15A NCAC 02H .1044(5)].

5. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other
documents. Pdfs must be uploaded using the form at:
https:Hedocs.deq.nc.gov/Forms/SW Project_Submittal
Please note that this request for additional information is in response to a preliminary review.
The requested information should be received by this Office prior to November 27, 2020, or
the application will be returned as incomplete. The return of a project will necessitate
resubmittal of all required items, including the application fee. Please reference the State
assigned project number SWA000079 on all correspondence.
If you need additional time to submit the information, please submit your request for a time
extension to the Division at the contact below. The request must indicate the date by which you
expect to submit the required information. The Division is allowed 90 days from the receipt of
a completed application to issue the permit.
The construction of any impervious surfaces, other than a construction entrance under an
approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject
to enforcement action pursuant to NCGS 143-215.6A.
