The physical address is the same:
158 N. Egerton Road, Mountain home, NC 28758.

I recently mailed a Change of Permit Owner Affiliation Designation form as well, but I did not see any form to change our
address or billing contact. If there is a form to change address or billing contact, please let me know so I can get that
completed.
