Dear Mr. Blackburn:
This letter is to confirm that a complete Engineer's Report and a Water System Management Plan have been
received, and that engineering plans and technical specifications have been approved for the water facilities to
serve Baylee Ridge subdivision, Johnston County Serial No. 20-065-P.
The Authorization to Construct is valid for 24 months from the Issue Date (September 18, 2020). Authorization to
Construct may be extended if the Rules Governing Public Water Supplies and site conditions have not changed.
Upon completion of the construction or modification, and prior to placing the new construction or modification
into service, the following information must be received and approved by the Johnston County Department of
Utilities:
1. Engineer's water certification
2. As -Built drawings sealed by the Engineer
3. Lien waiver from the utility contractor
4. Passing water sample test results (dated no earlier than 30 days prior to the County's acceptance of the
water facilities)
5. Recorded plat indicating any necessary utility easements
Please have the utility contractor schedule a pre -construction conference with our office prior to the installation
of the water distribution facilities. Please be advised that all permits must be received by this office prior to
