Please accept the enclosed permit renewal application for Michelin Aircraft Tire Company's
NPDES permit #NCS000373. All supplemental information has also been included with the
application.
MATC is committed to operating according to the stormwater permit. Unfortunately, we did not
complete qualitative sampling from September 2010 to September 2013 and the quantitative
monitoring was not completed from September 2010 to March 2014. These deficiencies were
reported to Mitch Gillespie on January 14, 2014. Since that time, actions have been taken to
prevent this from occurring in the future, and the semi-annual monitoring and reporting has been
performed as per the permit.

Retreading operation has returned to the Norwood facility. This process is being performed in what used to be the warehouse. A smaller
warehouse was built in 2013 for
the main purpose of storing semi -finished textile and this structure is attached to the south east side
of the plant. The small warehouse replaced the main parking lot. A new parking lot was constructed on the west side of the main plant
in 2013. An Employee Service Center was also built. All of these changes can be seen on the site map attached to this application.

SUPPLEMENTAL INFORMATION REQUIRED FOR RENEWAL OF INDIVIDUAL
NPDES STORMWATER PERMIT
Two copies of each of the following shall accompany this submittal in order for the application
to be considered complete:
(Do not submit the site Stormwater Pollution Prevention Plan)
Initials
1. A current Site Map from the Stormwater Pollution Prevention Plan. The location of
industrial activities (including storage of materials, disposal areas, process areas and
loading and unloading areas), drainage structures, drainage areas for each outfall,
building locations and impervious surfaces should be clearly noted.
2. A summary of Analytical Monitoring results during the term of the existing permit
(if your permit required analytical sampling). Do not submit individual lab reports.
The summary can consist of a table including such items as outfall number,
parameters sampled, lab results, date sampled, and storm event data.
3. A summary of the Visual Monitoring results. Do not submit individual monitoring
reports. The summary can consist of a table including such items as outfall number,
parameters surveyed, observations, and date monitoring conducted.
4. A summary of the Best Management Practices utilized at the permitted facility.
Summary should consist of a short narrative description of each BMP's in place at
the facility. If the implementation of any BMP's is planned, please include
information on these BMP's.
5. A short narrative describing any significant changes in industrial activities at the
permitted facility. Significant changes could include the addition or deletion of
work processes, changes in material handling practices, changes in material storage
practices, and/or changes in the raw materials used by the facility.
6. Certification of the development and implementation of a Stormwater Pollution
Prevention Plan for the permitted facility (Sign and return attached form).
If the final year analytical monitoring of the existing permit term has not been completed
prior to filing the renewal submittal, then the last years monitoring results should be submitted
within 30 days of receipt of the laboratory reports. (i.e. do not withhold renewal submittal
waiting on lab results)

Best Management Practices
The best management practices at the site include minimizing exposure, good
housekeeping, maintenance, spill prevention and response, erosion and sediment control, and
employee training.
To minimize exposure we perform routine observations at all of our pollutant sources.
There are no storm drains present in the immediate vicinity of any waste collection container.
Fuel oil storage tanks have concrete dikes to contain oil in the event of overlflow, tank rupture or
leakage. A procedure is in place to have an employee present during the unloading of oil to
ensure no spillage occurs and to cover each catch basin with an oil resistant mat to reduce
contamination if a spill does occur. There are additional spill control and containment equipment
located at the unloading stations. In the event of a large spill the outfall that receives stormwater
from these areas can be closed to reduce contamination to surface waters.
Good housekeeping practices are a part of Michelin's operations to maintain a clean and
orderly environment and thereby reduce the possibility of accidental spills and equipment
damage that may result in releases to surface waters. All employees are furnished with the
protective equipment necessary to complete tasks. Any small spills are removed quickly during
routine housekeeping and the housekeeping is audited daily by supervisors and quarterly by the
Environmental Coordinator.
Timely inspection and maintenance of stormwater management devices, facility
equipment and process systems are continuously performed to reduce the likelihood of failures
that could lead to discharge of pollutants to surface waters. All scheduled and non-scheduled
maintenance activities are recorded.
Employees are trained to identify and immediately report all spills and routine drills are
performed by responders. A Spill Prevention Control and Countermeasure plan has been
developed and implemented.
Vegetative practices, structural erosion prevention and sedimentation practices are
employed at the site. Where possible, natural vegetation is used, otherwise stabilization methods
are utilized. Several natural channels carry stormwater in a controlled manner to each outfall.
These channels are adequate to reduce flow velocity and control erosion. Any obvious erosion is
reported.
Plant personnel are trained on the proper operation and maintenance of equipment to
prevent oil discharges; discharge procedure protocols; applicable pollution control laws, rules
and regulations; and general facility operations. All employees receive general training during
new -hire orientation and annually. Facility maintenance technicians receive specific training that
pertains to their job functions.

Significant Changes in Industrial Activities
Since the last stormwater permit, there have been several significant industrial changes at
Michelin Aircraft Tires. The first change is the addition of the process of retreading aviation
tires. The retreading process is located in the building on the northwest side of the facility, which
used to be the tire warehouse. To accommodate this process, containers for the collection of
retread rubber dust as well as an additional dumpster for the collection of other retread
production wastes were added to the site. These containers are located at positions 1 and 9 on the
site map.
The second change is the addition of a warehouse to store semi -finished textile products.
This warehouse is attached to the main building and is located on the southeast corner of the
facility. The warehouse replaced most of the old parking lot so a new parking lot was added on
the west side of the main building.
The third change was the addition of an employee service center. The service center is a
small building southwest of the new warehouse. This building is used primarily for meetings and
also is the location for the on -site nurse.
