coverage under Certificate of Coverage Number NCG140193. In accordance with your request,
Certificate of Coverage Number NCG 140193 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of
stormwater to waters of the State without valid coverage under an NPDES permit is against
federal and state laws and could result in fines. If something changes and your facility would
again require stormwater or wastewater discharge permit coverage, you should notify this office
immediately. We will be happy to assist you in assuring the proper permit coverage.
If the facility is in the process of being sold, you will be performing a public service if you would
inform the new or prospective owners of their potential need for NPDES permit coverage.

Mineral, and Land Resources (DEMLR) is forwarding herewith the reissued Certificate of
Coverage (COC). This permit is reissued pursuant to the requirements of North Carolina
General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as
subsequently amended.) A new Certificate of Coverage (COC) is included with this
letter.
You must print a copy of the new NCG140000 General Permit from our website here:

permits/stormwater-permits des-industrial-sw. In addition to the full permit, the 2017
print package on the website includes revised Discharge Monitoring Report (DMR) forms,
Annual Discharge Monitoring Report forms, and Qualitative Monitoring Report Forms.
The General Permit authorizes discharges of stormwater and wastewater (if applicable),
and it specifies your obligations with respect to discharge controls, management,
monitoring, and record keeping. Please review the new permit to familiarize yourself with
all changes in the reissued permit. Significant changes to the General Permit are
outlined in the Technical Bulletin, which is also available on the website above. Your
facility has six months from receipt of the permit to update your Stormwater Pollution
Prevention Plan (SPPP) to reflect any new permit requirements.

How does the new General Permit affect Tier Status?
The first sample period of your permit begins August 1, 2017. Tier 3 actions in Part IV of
your permit are triggered by benchmark exceedances on four occasions beginning on the
effective date of this permit and do not count prior exceedances. Likewise, Tier 2 actions
are triggered by two consecutive benchmark exceedances beginning on the effective date
of this permit and do not count prior exceedances. However if your facility is already in
Tier 2 (monitoring stormwater discharges monthly) or Tier 3, you must continue monthly
monitoring until relieved through conditions of the permit or by DEMLR staff approval.
When does electronic DMR reporting start?
We are setting up our database with final permit parameters and the outfalls that
permittees submitted on-line. All NCG14 Permittees will receive notification when our
eDMR system is ready for these permittees to register and begin reporting
monitoring data electronically. If you have any questions about the status, please
contact Bethany Georgoulias at [phone] or Robert Patterson at [phone]. If
you did not submit outfall information already, we have included a form for you to return
to us. (Note: NO FORM IS ATTACHED if we already have your outfall information.)
Your coverage under the General Permit is transferable only through the specific action of
DEMLR. This permit does not affect the legal requirements to obtain other permits which
may be required by the N.C. Department of Environmental Quality (DEQ), nor does it
relieve the permittee from responsibility for compliance with any other applicable federal,
state, or local law, rule, standard, ordinance, order, judgment, or decree.

I. DIRECTIONS TO SITE: From From FRO, take Hwy 401 South to Wagram. Turn right at Old Wire
Road (Hwy 144) and continue —11 Yz miles to Laurel Hill. Turn right onto Hwy 74 West and travel —35
miles to Wadesboro. Continue through town, the facility is on the left, —50 yards past the Hwy 52
intersection.
IL SAFETY: Standard FRO safety gear is required.
M. FACILITY DESCRIPTION: Concrete Supply Co., LLC is a dormant concrete batch plant located in
Wadesboro, Anson County. The facility has not been operational for years, but there are still silos,
conveyors and raw material bunkers on site. The facility's air quality permit was rescinded on 09 August
2016 under the newly adopted 2Q .0102(d) exemption.
IV. INSPECTION SUMMARY: On 07 September 2016,1 Joshua Harris, of FRO DAQ conducted a site
visit for a multimedia inspection of the Concrete Supply Co., LLC., - Wadesboro location. This facility
remains dormant, and there are no stockpiles on site, nor is there any evidence of production activity.
According to Mr. Johnie Alexander, Environmental Manager, there are no immediate plans to operate at
this site.
V. NON-COMPLIANCE IIISTORY SINCE 2010: The facility does not have a history of non-
compliance.
VI. RISK MANAGEMENT (112r): This facility does not use or store any 112(r) subject materials in excess
of threshold quantities. Therefore, the facility is not required to maintain a written Risk Management
Plan (RMP).
VH. CONCLUSION AND RECOMMENDATIONS:
Concrete Supply Co., LLC - Wadesboro was non -operational at the time of the visit, and appeared to be
IN COMPLIANCE with applicable air quality regulations.

As you know an initial site visit was performed at the CEMEX plant in Wadesboro, NC on
September 9, 2011 by Paul Rawls of the Fayetteville Regional Office to verify that no industrial
activity was taking place. At that time it was observed that chemical additives were still being
stored on the site. A second and third visit was conducted October 5, 2011 and February 14, 2012
respectively with the same outcome. A final visit was conducted March 2, 2012.after you informed
Mr. Rawls that the chemicals had been removed. That visit revealed that in fact the chemical
additives had been removed. Your request to keep the NCG140OOO permit (COC NCG14O193), but
to discontinue stormwater and wastewater monitoring is hereby approved, based on the following
conditions:
There shall be no industrial activities taking place, no stockpiles of raw materials (to include
chemical additives) stored on the site, or wastewater or stormwater industrial discharges
while the permittee is not monitoring.
No wastewater shall be generated, and all wastewater treatment units shall be cleaned,
stabilized, and inactivated as to render them harmless to the environment while the site is
dormant.
Qualitative and analytical monitoring actions as described in Part IV, Sections A, B, C, and
D do not have to be conducted - as long as the above conditions and all other permit
conditions and limitations are met, including:
The annual permit fee must be paid. Failure to pay the annual permit fee will be
considered a violation of the subject permit.

The Stormwater Pollution Prevention Plan (SPPP) shall be kept updated to reflect the status of the
facility. Updates and conditions of the SPPP of note are:
The BMP Summary shall be reviewed and updated annually.
Training programs shall be developed and training provided at least once a year
for facility personnel with responsibilities for: spill response and cleanup,
preventative maintenance activities, and for any of the facility's operations that
have the potential to contaminate stormwater runoff.
All aspects of the Stormwater Pollution Prevention Plan shall be reviewed and updated on an
annual basis. The annual update shall include an updated list of significant spills or leaks of
pollutants for the previous three years, or the notation that no spills have occurred. The annual
update shall include written recertification that the stormwater outfalls have been evaluated for the
presence of nonstormwater discharges. (Should nonstormwater discharges be discovered, the
permittee shall contact this office as soon as possible but in no case greater than 24 hours or the
next business day).
Inspections of the facility and all stormwater systems shall occur as part of the Preventative
Maintenance and Good Housekeeping Program at a minimum on a semi-annual schedule.
This office must be notified in writing at least 30 days prior to commencing any industrial activity
on the site, even if updates or changes are temporary.
A copy of this letter must be maintained with the facility permit and associated plans and records
for the life of the permit.

modification is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the U. S. Environmental Protection Agency
dated December 6, 1983.
This permit does not affect the legal requirement to obtain other permits which may be required by
the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act, or any other Federal or Local government permit that may be required.

Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
* A copy of general Stormwater permit NCG140000
y A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin for the general permit which outlines changes in the permit, key requirements,
and addresses frequently asked questions
* A Certificate of Coverage
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.

In accordance with your application for a stormwater discharge permit received on April 3, 1995, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state = NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
The stormwater permit you are receiving, NCG140000, does not permit wasting concrete, dumping excess
concrete, directly into storm sewer outfalls or into waters of the state. Any facility which either dumps excess
concrete or washes excess concrete into storm sewers or waters of the state will be operating in direct violation to
both the terms of this permit and the North Carolina General Statutes. Such a discharge shall be considered an
illegal discharge and may subject the owner to enforcement actions in accordance with North Carolina General
Statutes 143-215.6A
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to request an individual permit by submitting an individual permit application. Unless
such demand is made, this certificate of coverage shall be fmal and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of
Environmental Management. The Division of Environmental Management may require modification or revocation
and reissuance of the certificate of coverage.
This permit -does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
