The assistance provided by Mr. Jay Campbell, Plant Manager, and Mr. Tony Sample, Compliance Manager, during the inspection was greatly appreciated. The inspection report is attached. Findings during the inspection were as follows: 1. Certificate of Coverage (COC) No. NCG020438 under NPDES General Permit NCG020000 was issued to Carolina Sunrock LLC — Kittrell Quarry to discharge stormwater runoff and process wastewater to an unnamed tributary to Tar River, a WS-IV, NSW stream, in the Tar - Pamlico River Basin. 2. The facility has developed a stormwater pollution prevention plan (SPPP). A review of the plan indicated that it included all components required by the General Permit. Semi-annual qualitative monitoring has been conducted as required by the permit at the all five stormwater discharge outfalls (SDOs). Stormwater Best Management Practices (BMPs) have been inspected weekly. Sediment accumulation was noted in the settling basins at SDO 1, and SDO 2, which significantly reduces the capacities of both basins. Sediment in these basins needs to be removed periodically to ensure that they function as designed. 4. Carolina Sunrock — Kittrell Quarry was found to have violated Part III, Section B, of the General Permit for failure to perform analytical monitoring for Stormwater discharge from one SDO (Pond 1). The facility has been reporting no flows at all SDOs. Instead of inspecting the risers, semi- annual qualitative monitoring at this outfail shall be performed at the ends of discharge pipes. 5. All five SDOs were observed during the inspection.
