A Compliance Evaluation Inspection performed at the Greenlee Pit on May 23, 2106, by personnel from this office
verified that no industrial activity was taking place at the time of the inspection. Your recent request to keep the
NCG020231 permit, but to discontinue stormwater monitoring is hereby approved based on the following conditions:
There shall be no industrial activities taking place, no stockpiles of raw materials (to include chemical
additives) stored on the site, or wastewater or stormwater industrial discharges while the permittee is not
monitoring.
No wastewater shall be generated and all wastewater treatment units, if any, shall be cleaned and stabilized,
and inactivated as to render them harmless to the environment while the site is dormant.
The Stormwater Pollution Prevention Plan (SPPP) shall be kept updated to reflect the status of the facility.
Updates and conditions of the SPPP of note are:
o The BMP Summary shall be reviewed and updated annually.
o Training programs shall be developed and training provided at least once a year for facility
personnel with responsibilities for: spill response and cleanup, preventative maintenance
activities, and for any of the facility's operations that have the potential to contaminate
stormwater runoff.
o All aspects of the Stormwater Pollution Prevention Plan shall be reviewed and updated on an
annual basis. The annual update shall include an updated list of significant spills or leaks of
pollutants for the previous three years, or the notation that no spills have occurred. The
annual update shall include written recertification that the stormwater outfalls have been
evaluated for the presence of non-stormwater discharges. (Should non-stormwater discharges
be discovered the permittee shall contact this office as soon as possible but in no case greater
than 24 hours or the next business day).
o Inspections of the facility and all stormwater systems shall occur as part of the Preventative
Maintenance and Good Housekeeping Program at a minimum on a semi-annual schedule.
Qualitative and analytical monitoring actions as described in Part I11, Sections A, B, C,1), and E do not
have to be conducted — as long as the above conditions and all other permit conditions and limitations are met,
including:

o The annual permit fee must be paid. Failure to pay the annual permit fee will be
considered a violation of the sub'ect permit.
This office must be notified in writing and all components of the stormwater permit requiring updates made at
least 30 days prior to commencing any industrial activity on the site, even if they are temporary.
A copy of this letter must be maintained with the facility permit and associated plans and records for the life
of the permit.

Inspection Summary:
I inspected the site with Jason Conner. Inspection was to determine dormancy status.
The Greenlee Pit is not active and had no activity occuring. The site is being monitored and maintained. On a previous
inspection I had suggested that Jason request dormant status to discontinue stormwater monitoring. We received this
request on April 28, 2016. Their mining permit is current and they are in compliance with DLR.
We discussed that the annual stormwater permit fees will still need to be paid, and if at any time the mine wants to start
back up a request would need to be submitted to return to active status.
I recommend granting dormant status for the mine.

Enclosed please find a copy of the Reconnaissance Inspection form from the
inspection conducted on February 27, 2006. The Reconnaissance Inspection was
conducted by Starr Silvis and Laurie Moorhead of the Asheville Regional Office.
The facility has not been actively mined in at least five years according to Jason
Conner. Mr. Conner's assistance during the inspection was much appreciated. At the
time of the inspection the majority of the site appeared to be reclaimed and heavily
vegetated. The only activity at the site is the hauling of stockpiled rock off site. The
facility has two potential options with regards to compliance with the NPDES stormwater
permitting program:
1) Review the enclosed application for a certificate of no exposure. If it
is determined that a certificate of no exposure applies to the site,
complete the form and submit the application to the indicated
address.
2) Review the enclosed application for a certificate of no exposure. If it
is determined that a certificate of no exposure does not apply to the
site, then the facility is required to adhere to the current permit
conditions contained in NCG020231. Compliance with said permit
will require preparation of a Stormwater Pollution Prevention Plan as
well as monitoring at all outfalls.

The facility has been inactive for five to six years. The only activity on the site is removal of a stockpile of
clean gravel. The majority of the site has been reclaimed and is heavily vegetated. There is one small
staging area that has exposed soil. Runoff from the staging area appears to flow into a ditch for which an
outlet to the Catawba River was not found. The facility may be eligible for a certificate of no exposure. The
