In accordance with your request received October 26, 1998, the Division is forwarding the subject
permit. The changes in this permit are only with regard to a name and an ownership. All other terms and
conditions in the original permit remain unchanged and in full effect. This permit modification is issued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina and the U. S. Environmental Protection Agency dated December 6,
1983.
This permit does not affect the legal requirement to obtain other permits which may be required by
the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act, or any other Federal or Local government permit that may be required.
The application fee that you submitted is being returned herein. As of January 1, 1999, the fee
structure for our permits has been revised to and no longer requires a fee for these changes.

Form for General Wastewater Permit No. NCG520047. The required legal documentation of the
transfer of ownership (i.e., contract, deed, articles of incorporation) was not submitted. The request is
considered incomplete. Therefore, the Division of Water Quality is not able to process your request until
the required information has been submitted.
Your name change form and check are being returned to you herein for resubmittal with the appropriate
signatures and documentation.

The Division of Water Quality received your request for a Name/Ownership Change for the
subject facility on January 26, 1999. Your request package included a check for $100.00 to cover the
minor modification fee. Recent legislative changes have resulted in changes to the permit fee structure
for all of the permits handled by the Division of Water Quality. Under the new fee structure there is no
fee required for minor modifications to a permit.
In accordance with this fee change we are returning your check in the amount of $100.00. We
