Skvarla, III, Secretary April 3, 2014 Subject: Notice of Deficiency NOD-2014-PC-0078 Sysco Food Service of Raleigh NPDES Stormwater Permit NCG080819 Johnston County On March 21, 2014 Thad Valentine, from the Raleigh Regional Office of the North Carolina Division of Energy, Mineral and Land Resources (DEMLR) conducted a compliance evaluation inspection (CEI) at your facility, located at 1032 Baugh Road, Selma, North Carolina. The following observations were noted during the DEMLR inspection and file review: This facility is a Food Service Company with a'permit renewal on December 1,.2012 which will expire on November 31, 20.17. This facility has on -site tanks and drums utilized for changing oil, waste oil from oil filters, petroleum above ground storage tanks for fueling trucks and Urea used as a diesel Exhaust Fluid. The petroleum tanks, Urea tank and waste oil from used oil filters are located outside, the diesel fuel being the only one with dual containment and the other two needing dual containment. Stormwater run-off generated at such facilities is regulated by general NPDES stormwater permit NCG080000. The DEMLR Stormwater Program has issued certificate of coverage (COC) 'NCG080819 and the permit was on site and current. The Stormwater Pollution Prevention Plan (SPPP) was reviewed by DEMLR staff. The plan lacked the following: Qualitative monitoring, analytical data from the vehicle maintenance area, and a document for yearly spills significant or other- for the last three years. During the last inspection on March 30, 2011 no analytical or qualitative data collection was being performed. For this inspection analytical data was included in the Stormwater Pollution Prevention Plan but no Qualitative data collection was present. The sample was taken from the storm drain outlet pipe located at the inlet to the retention pond which accounted for the storm drain run off only. Containment and storage of oils, fuels and other liquids were inadequate, with respect to secondary containment. DEMLR Staff and Sysco Food Service of Raleigh staff discussed issues with lack of dual containment for the waste oil filter disposal and the possible need for dual containment for the '1200 gallon Urea (Diesel Exhaust Fluid Mixture) located at the fuel pumping area. Also discussed by phone the day of the inspection was the outlet area for the oil water separator. Staff from Sysco Food Service of Raleigh indicated that samples have not been taken from the outlet from the oil water separator Requested response: You are directed to respond to this letter in writing to DEMLR at the address provided below within 30 days of receipt. Please address the following items noted in bold: Please fully develop the Stormwater Pollution Prevention Plan by: • Placing a list of significant spills during the last three years in the plan • Documenting the qualitative monitoring from sample point which will include the outlfall for the oil water separator • Document the analytical data collected including the outfall from the oil water separator • Provide secondary containment for the Urea (Diesel Exhaust Fluid Mixture) or otherwise contain the Urea in a vessel sized to comply with the permit 1628 Mail Service Center, Ralegh. Comment: The plan did not include any notations for yearly spill documentations even though they had no spills There was no dual containment for a oil filter disposal bin located outside a maintanance area and there is a tank containing Urea and water that could be over the size limit for single wall containment. The Urea tank is larger then 660 gallons so it does require dual containment or a tank less then 660 gallon of capacity. Your assistance with this inspection was greatly appreciated. Below is a list of findings and recommendations developed from the inspection: The subject permit was issued and became effective April 18, 2008 and expires October 31, 2012. Permit renewal is required 180 days prior to permit expiration. 2. Sysco is a national distributor of food products and kitchen, office and cleaning supplies and equipment, smallwares and disposables. The facility contains refrigerated and freezer spaces and dry goods storage areas. All materials are shipped by freight (tractor trailers). 3. Industrial activities at the facility consist of a vehicle maintenance and truck washing area, and a fueling station for trucks. The fueling station consists of 2 — 10,000 gallons Above Ground Storage Tanks (AST's) with an oil water separator equipped with a petro vend auto spill monitoring system. Waste oil and grease are stored inside the maintenance area. The truck washing area also contains an oil water separator. Eastern Environmental Management pumps out waste oil for further processing off site. 4. There are three stormwater drainage basins listed on the site map in the Stormwater Pollution Prevention Plan (SPPP). Drainage area 1 consists of stormwater from rooftop, parking lot drainage from the loading and unloading area on the north side of the building, and the vehicle maintenance area which includes the truck washing area. Stormwater from this drainage area is routed to stormdrains and conveyed to a large stormwater pond�with an overflow pipe on the north side of the property. Water from the overflow pipe discharges at Stormwater Discharge Outfall (SDO) 1 to an Unnamed Tributary to Mill Creek. Discharge from the overflow appeared clear at the time of inspection. • Drainage area 2 consists of stormwater from a 2 acre area that drains to a dry basin on the south side of the building and a portion of rooftop drainage. This dry basin discharges at SDO 2 and drains to drainage area 3. There was no discharge at the time of inspection. This dry basin discharges at the edge of the road, SDO 3, flows under Baugh Road and into the road ditch. There was no discharge at the time of inspection. 5. Discharges for SDO 2 and SDO 3 are not associated or comingle with industrial activities on site and therefore do not require monitoring. You may remove SDO 2 and SDO 3 from your permit monitoring requirements. 6. The current stormwater permit was available for review. All elements of the SPPP except monitoring are being implemented. You stated that you were not aware analytical and qualitative monitoring were a requirement in the permit. You stated that monitoring will begin as soon as the first opportunity arises. Please note that not adhering to permit requirements is subject to penalty enforcement. You must follow test procedures approved under federal regulations in 40 CFR §136 when analyzing pH. The parameter pH 'is required to be measured within 15-minutes after taking a sample. All laboratories analyzing other discharge characteristics must be NC certified. We have received an application for coverage of an existing facility under NCG 08 from Sysco Food Services of Raleigh. The facility drains to Mill Creek (class WS-IV; NSW) in Selma (Johnston County).
