You were present for this inspection which evaluated the areas designated on the attached EPA inspection form. Observations from each area are addressed below: 1. Permit This facility holds General Stormwater Permit No. NCG070181 to discharge Stormwater from activities associated with Stone, Clay, Glass & Concrete Products under the National Pollutant Discharge Elimination System (NPDES). The permit became effective June 1, 2013 and expires May 31, 2018. 2. Records/Reports - Part II, Section A of the permit requires the development of a Stormwater Pollution Prevention Plan (SP3). This had not been accomplished at the time of the inspection. Records show that the Certificate of Coverage for this site was issued April 27, 2009. The SP3 plan should have been in place and complete at that time and updated yearly since. A portion of the inspection was used going over the SP3 plan and what was to go in it. A follow-up inspection will be scheduled to review the plan once it is in place. 3_ Facility Site Review Site has improved from the inspection of March 2009. There are still items being stored outside that should be covered. Any items that have potential to carry leachate or solids off site during storm events must be stored under cover. 4. Effluent/Receiving Waters The facility's outfall discharges into an unnamed tributary to Hanks Creek, a class C water in the Yadkin Pee -Dee River Basin. Self -Monitoring Program Analytical monitoring is not required at this site per Part 11, Section B of the permit since the facility uses < 55 gallons/month of new motor oil. The semi-annual qualitative (visual) monitoring in Part II, Section C is required but is not being performed and documented on a semi-annual basis. The proper documentation practices were also discussed during the inspection. Sue White feels that Finch Industries is on board and will do the required monitoring from this point on. Work began on the pit cover April 16, 2009 by Triangle Construction. Enclosed is a letter from them giving a completion date of 4/23/2009. 2. A copy of the Notice of Intent that was submitted is enclosed. 3. The waste from the outdoor pit was placed in our dumpster and taken to the lined county dumpsite. This dumpster is serviced by Best Disposal in Thomasville, North Carolina. 4. The pit contains a synthetic coolant, water, glass grinds and a small amount of cerium oxide with grinding wheel residue. A diagram is enclosed of how the system is piped. 5. The pipe is a basically a closed loop. The pit will be drained and cleaned 3 times a year depending on usage. Before cleaning, the water goes into the Thomasville sewage system and then the solid residue will go into the dumpster. 6. Your office will be notified when the new permit is received. (SIC code 3231) 104 Williams Street, Thomasville, NC Davidson County Dear Mr. Ingram: On March 3, 2009, 7enifer'Carter, of the Division of Water Quality, performed a stormwater inspection of Finch Industries in Thomasville, North Carolina. You were present for the inspection. The inspection was initiated due to concerns of staff at the City of Thomasville about industrial waste overflowing from an outdoor pit into the stormdrain system. Observations: + Process waste was overflowing from the pit during the pre -arranged inspection. You stated that the open, uncovered pit contains process waste that is piped to it from inside the building. You said it is sometimes recycled, and sometimes removed and hauled off. Overall housekeeping outdoors was very poor. There is an open dumpster containing office and industrial waste,. including broken and finely crushed glass. The dumpster is in a shallow retaining area, though waste has been allowed to accumulate around it and a nearby culvert. A separate mound of finely crushed glass was on site, you said, due to lack of room for it in the dumpster. + The entire site drains to an unnamed tributary to Hanks Branch (class C. waters). You are hereby advised that the illegal discharge of wastewater is a violation Forth Carolina General Statute (NCGS) 143-215.1(a), which states, "No person shall do any of the following things or carry out any of the following activities until or unless such person shall have applied for and shall have received from the Commission a permit...
