inspection Action Code FCE On -Site Inspection Result Compliance TSP S02 NOX VOC CO PM10 * HAP 2013 03001 00000 00009 00001 00008 0 1400 00833 2008 05200 --- --- --- --- 02400 0 1438 Five Year Violation History None Date Letter Type Rule Violated Performed Stack Tests since last FCE None Date Test Results Test Method(s) * Hip -hest HAP Emitted (in Violation Resolution Date Source(s) Tested Permitted Sources Emission FEmission Source Description Control Control System Description Source ID System ID One truck mix concrete batch plant (120 cubic yards per hour maximum rated capacity), consisting of ES-1 F Cement silo (113 tons capacity) CD 1 square feet of Fabric filter filter filter area) area) ES-2 F Cement silo (80 tons capacity) ES-3 Flyash silo (50 tons capacity) ES-T-F Weigh hopper ES-5-1 Truck loadout Insignificant/ Exempt Sources Source ID I Source Description Exempting Source Source of Regulation of Title V TAPS Pollutants Sources Appearing on the Permit Attachment IES-9 1 4 million Btulhr natural gas process water heater [2Q 0102(c)(2)(B)(u) No Introduction On August 17, 2016, Mr Charles Gerstell, NCDEQ Multimedia Inspector, contacted Mr Howard Hepler, Plant Manager, and Mr Danny Rathbone, Operations Manager, of Ready Mixed Concrete Co — High Point for an announced multimedia compliance inspection The facility was previously inspected on December 4, 2014 by Mr Paul E Williams of the Winston-Salem Regional Office and was found to be in compliance with Air Quality standards and regulations at that time The facility is a concrete batching and truck -mix facility that currently operates 8 hours a day, five days a week, and 45-52 weeks per year, depending on production demand All IBEAM contacts were checked and Mr Rathbone's title was changed from Division Manager to Operations Manager upon his request Safety Equipment Safety shoes, eye protection, hard hat and safety vest are required within manufacturing areas Latitude/Longitude Verifications The latitude and longitude coordinates were verified to be accurate as documented in IBEAM Applicable Regulations According to permit Condition A 1, the facility is subject to the following regulations Title 15A North Carolina Administrative Code (NCAC), Subchapter 2D 0202, 2D 0515, 2D 0521, 2D 0535, 2D 0540, 2D 0611, 2D 1806 and 2Q 0309 The facility is not §ubject to the RMP requirements of the i 12(r) program since it does not use or store any of the regulated chemicals in quantities above the threshold in the rule The facility is only subject to the General Duty clause of the Section 1 12(r) program Discussion Ready Mixed Concrete Company — High Point is a truck mix concrete batch plant that operates two cement storage silos (ES-1, ES-2), flyash silo (ES-3), weigh hopper (ES-4) and truck loadout (ES-5) Particulate matter from these sources is controlled by bagfifter unit (CD-1) The facility has four aggregate bins that are transferred with a front-end loader The materials are then weighed and dispensed, in measured amounts, via a conveyer that carries them to the weigh hopper Flyash and cement are then dispensed from their silos into the weigh hopper Designated amounts of water and additives are then administered prior to the concrete being dropped into a mixing truck r During the inspection, the inspector observed one concrete truck being loaded Minimal dust was emitted during loading and dissipated before reaching the property boundary A "skirt" was observed which is associated to the truck loadout (ES-5) At the time of the inspection, the skirt was worn and in need of attention Mr Rathbone explained that the wear on the device was the result of trucks loading and rubbing the skirt It was recommended that the skirt be repaired to aid in controlling dust from the loading process A magnehelic gauge was mounted adjacent to the ladder of the bagfilter unit (CD-1) which normally operates between 2 5 and 3 inches of water column according to Mr Hepler Mr Hepler also stated that production to date (this calendar year) was 7,783-cubic yards Permit Conditions/ Applicable Regulations Condition A 2 contains 15A NCAC 2D 0202 permit renewal and emissions inventory requirements The permit expires October 31, 2022 and the facility is required to submit a renewal request and air pollution emission inventory report for the 2021 calendar year 90 days prior to the expiration of the permit This condition was discussed with Mr Hepler and Mr Mr Rathbone Compliance is anticipated Condition A 3 contains the 15A NCAC 2D 0515 particulate control requirement This condition sets the maximum allowable particulate emissions for concrete batching using the following two equations E = 4 10 * (P) ° 61 for P <= 30 tons/hr , or E = 55 * (P) ° � i - 40 for P >30 tonslhr Compliance is expected since the bagfilters appear to be properly maintained and in good working order Condition A 4 contains the 15A NCAC 2D 0521 visible emissions control requirement This facility is limited to 20% opacity visible emissions when averaged over a six -minute period No visible emissions were detected from the bagfilter devices during this inspection Compliance with this requirement is indicated Condition A 5 contains the 15A NCAC 2D 0535 rule which requires the permittee to notify the director of any excess emissions lasting longer than four hours resulting from a malfunction, a breakdown of process or control equipment or any other abnormal conditions Mr Hepler and Mr Rathbone stated that no malfunctions had occurred since the last inspection and there are no issues registered to this facility in the (BEAM database indicating compliance with 0535 Condition A 6 contains the 15A NCAC 2D 0540 fugitive dust control requirement Majority of the site is surfaced with concrete Facility staff stated that aggregate and work areas are wetted with water when needed to control dust on site According to the DAQ ]BEAM database, no complaints have been received regarding fugitive dust emissions at this facility and minimal fugitive dust was observed during this inspection, with none leaving the boundary of this facility Compliance with 2D 0540 is expected Condition A 7 contains the 15A NCAC 2D 0611 fabric filter inspection, maintenance, and recordkeeping requirements for particulate matter emissions control The facility is required to perform, at a minimum, an annual (for each 12-month period following the initial inspection) internal inspection of the fabric filter system and perform periodic inspection and maintenance as recommended by the equipment manufacturer The facility is required to record the results of all inspection and maintenance activities in a logbook Mr Hepler presented bagfilter maintenance and production records to this inspector These records indicated the facility performs routine inspections on a weekly basis and internal inspections on a monthly basis The most recent internal inspection was performed on August 5, 2016, while the most recent routine inspection was performed on August 16, 2016 Compliance with this condition is expected Condition A 9 contains the 15A NCAC 2D 1806 control and prohibition of odorous emissions requirements No objectionable odors were observed originating from the facility at the time of inspection The facility does not have any previous odor complaints in IBEAM database Compliance with this condition is expected NSPS/NESHAP This facility does not appear to be subject to any NSPS or NESHAP rules at this time Facility Wide Emissions Actual emissions from permit review R08 for Ready Mixed Concrete Co — High Point (Read Plott, November 10, 2013) o Pollutant L Actual Emissions tual, Uncontrolled Emissions (tp F P I{ : g 8 },.. �„ ��,�€ t (y) _ a.-.4 §k..�.., ..� t ' S.Idi—f PM 030 5 7 PM10 014 22 SO2 5 6 106 _ --- -
