Skvarla, III, Secretary February 12, 2014 Subject: General Stormwater Permit Inspection NPDES Permit No. NCG110018 High Point Westside WWTP, Davidson County Dear Mr. Houk: On February 11- 2014, Sue White of this office performed a Stormwater Permit Inspection at High Point Westside WWTP located at 1044 West Burton Road, Thomasville, NC in Davidson County. John Hodges and Tim Fitzgerald of the Plants Division and Justin Gray of the Stormwater Division were present for the inspection. This facility holds General Stormwater Permit No. NCG110018 to discharge Stormwater from activities associated with Treatment Works under the National Pollutant Discharge Elimination System (NPDES). 1. Permit The permit became effective June 1, 2013 and expires May 31, 2018. 2. Records/Reports During the inspection, it was determined that your facility has a developed Stormwater Pollution Prevention Plan (SPPP). All required information was included in this plan. 3. Facility Site Review This site is a > 1 MGD municipal wastewater treatment plant. The site is well cared for and it is obvious that the owners and employees implement permit requirements. This facility has four outfalls identified in the SPPP. 4. Effluent/Receiving Waters Stormwater from the property discharges to Rich Fork Creek (Class C waters) in the Yadkin/Pee Dee River Basin. 5. Self -Monitoring Program It was determined during the inspection that you have been performing the self -monitoring requirements of the General Permit. Monitoring requirements are for qualitative monitoring at each stormwater discharge outfail on the property only. Analytical monitoring is not required at this site. Phone: 336-771-50001 FAX: 336-771-4631 High Point Westside WWTP 2/12/2014 Page 2 of 2 The records inspection revealed that while pumping out secondary containment, fluids from the containment area with a pH of >9 were released into Rich Fork Creek on more than one occasion. As you know, all discharges to the creek must be within stream standards, which for pH is between 6-9. It is recommended that training be provided to the Westside Staff to prevent this from occurring in the future. All other factions of this inspection were satisfactory. This office appreciates the approach Mr. Fitzgerald and his staff takes in maintaining compliance at this site. This permit expires on March 31, 2003. The Division staff is currently in the process of rewriting this permit and is scheduled to have the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for continued coverage under the reissued permit. In order to assure your continued coverage under the general permit, you must apply to the Division of Water Quality (DWQ) for renewal of your permit coverage. Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial facilities in any of the 1 I categories of "storm water discharges associated with industrial activity," (except construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. This permit expires on March 31, 2003. The Division staff is currently in the process of rewriting this permit and is scheduled to have the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for continued coverage under the reissued permit. In order to assure your continued coverage under the general permit, you must apply to the Division of Water Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal Application Form. The application must be completed and returned by October 2, 2002 in order to assure continued coverage under the general permit. Due to staff and budget constraints, letters confirming our receipt of the completed application will not be sent. Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00. Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could result in assessments of civil penalties of up to $10,000 per day. Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial facilities in any of the I 1 categories of "storm water discharges associated with industrial activity," (except construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion.
