On February 5, 2013, Jenifer Carter, of the Division of Water Quality, performed a stormwater inspection at
Shelba D Johnson Trucking, Incorporated, located at 1640 Blair Street in Thomasville, North Carolina You were
present during the inspection The following observations and/or permit conditions were noted during the
inspection
A Stormwater Pollution Prevention Plan (SPPP) has been developed in accordance with the permit
➢ A new bulk terminal needs to be added to the SPPP's site plan and map Document any needed changes to
the SPPP, or the lack thereof annually
➢ Annual staff training needs to be documented
Analytical monitoring has been conducted in accordance with permit at four (4) outfalls Vehicle maintenance is
performed onsite Since the last inspection, no data has exceedences benchmark values Submit analytical data
within 30 days _ of receiving results to the address noted on the Discharge Monitoring Form (DMR) If any
benchmark values are exceeded, follow the tiered response requirements in the permit
• Qualitative monitoring has been conducted at all outfalls, and recorded in accordance with the permit

Prevention Plan (SPPP) First, I would Iike to acknowledge Ms Jenifer Carter and her
knowledge during her inspection of our facility Please know that our failure to create a site -
specific SPPP was purely out of lack of knowledge We were unaware of the North Carolina
Storm Watei policies and monitoring requirements Ms Carter has offered her assistance ill any
way possible and we appreciate that as well as your office cooperating with us Due to the
seriousness of this matter, we had contracted with Operations Excellence on this project and ale
confident Of t we will be able to develop and incorporate the plans and monitoring operations to

inspection at Shelba D Johnson Trucking, Incorporated, located at 1640 Blair Street in Thomasville,
North Carolina You were present during the inspection The following observations and/or permit
conditions were noted during the inspection
A Stonnwater Pollution Prevention Plan (SPPP) has not been developed in accordance with the permit
(Part 11 Section A) A genene company -wide SPPP was available, but was not site -specific
Analytical monitoring has not been conducted in accordance with permit Vehicle maintenance is
performed onsite, so exceedences of benchmark values require you to respond with increased
monitoring, actions and record keeping (Part II Section B) All analytical data must be submitted as
per Part III Section E)
• Qualitative monitoring has not been conducted and recorded in accordance with the permit (Part II
Section D)
Other Observations
• Alter a walk of the perimeter of the property, at least three outfalls were observed
There are not aboveground storage tanks with the capacity (individually or collectively) requiring
secondary containment
Requested Response, due within 15 business days of receipt of this letter

This letter is to inform you that as of the date of this letter the Division of Water Quality has not received a
renewal request for the subject permit certificate of coverage This is a viol ►tion of NCGS §143 215 1 (c)(1) which
states All applications shall be filed with the commission at least 180 days in advance of the date on which it is
desired to commence the discharge of wastes or the date on which an existing permit expires, as the case may be
Any permittee that has not requested renewal at least 180 days prior to expiration or permittee that does not h ive a
permit alter the expiration and has not requested renewal at least 180 days prior to expiration will be subjected to
enforcement procedures is provided in NCGS § 143-215 6 and 33 USC 1251 et seq
In order to prevent continued e%Lalaied action, including the assessment of civil penaltieti you must submit a
completed permit coverage renewal application to the attention of the Stormwater ind General Permits Unit at
the letterhead address within ten (10) days of your receipt of this letter (renewal application enclosed) It the
subject discharge has been terminated please complete the enclosed rescission request form Mailing instructions
are listed on the bottom of the form You will be notified when the rescission process has been completed
