All notices of intent to be covered under this General Permit shall be signed as follows: (1) For a corporation: by a responsible corporate officer. For the purpose of this permit, a responsible corporate officer means: (a) a president, secretary, treasurer or vice president of the corporation in charge of a principal business function, or any other person who performs similar policy or decision making functions for the corporation, or (b) the manager of one or more manufacturing production or operating facilities employing more than 250 persons or having gross annual sales or expenditures exceeding 25 million (in second quarter 1980 dollars), if authority to sign documents has been assigned or delegated to the manager in accordance with corporate procedures. (2) For a partnership or sole proprietorship: by a general partner or the proprietor, respectively; or (3) For a municipality, state, federal, or other public agency. by either a principal executive officer or ranking elected official. Shallotte Ready -Mix Facility 197 Barnhill Road NW, Supply, NC 28462 Sir/Madam- Thoma. CONCRETE NOV 1S1019 DENR-LAND QUALITY STORMWATER PERMITTING Below is supporting narrative and related information for NOI for NCG140000. The Shallotte Ready -Mix Facility or Concrete Plant is a dry batch plant which depends upon the ready mixed concrete trucks to mix the concrete. Component materials are discharged into the ready mixed concrete truck(s) for delivery to the job site. The plant consists of aboveground aggregate bins, cement, and fly ash silos and conveying and weighing equipment. The locations of industrial activities are illustrated on the site plan. Fine and coarse aggregates are delivered by trucks and stockpiled onsite. Materials from stockpiles are loaded into the respective hopper and conveyed to the concrete batch plant. Cement and fly ash are delivered to the site by tanker trucks and pneumatically pumped into the overhead silos. The truck mixer and silos have baghouses which minimize dust emissions during unloading and batching operations and minimize exposure of the cement particles to stormwater. Admixtures used in the concrete are delivered to the site in tanker trucks and pumped into and stored in bulk chemical storage tanks. All tanks are located within a conex container. Admixtures are then pumped into the concrete in varying proportions depending upon the mix design requirements. The cement, fly ash, sand, stone, water, and admixtures are proportioned by weight (by volume) and discharged into the truck mixer for mixing. A rinse/wash water recycle pit system is available at this facility. All of the runoff from this site is tributary to a wet pond constructed for the building project.
