Industry Drive Oxford, NC 27565 NORTH CAROLINA Environmental Quality November 11, 2019 Subject: COMPLIANCE EVALUATION INSPECTION Bridgestone Bandag, LLC Bandag-Oxford Facility, Certificate of Coverage-NCS000281 Granville County Mr. Battle: On November 7, 2019 a site inspection was conducted for the Bridgestone Bandag, LLC facility located at 505 West Industry Drive, Granville County, North Carolina. A copy of the Compliance Inspection Report is enclosed for your review. The site visit and file review revealed that the subject facility is covered by the Certificate of Coverage NCS000281. Permit coverage authorizes the discharge of stormwater from the facility to receiving waters designated Fishing Creek, a Class QNSW waterbody in the Tar Pamlico River Basin. As a result of the inspection, the facility was found to be in compliance with the conditions of the NCGO00281 permit. Please refer to the enclosed Compliance Inspection Report for additional comments and observations made during the inspection. Please be advised that violations of the NPDES Stormwater Permit are subject to a civil penalty assessment of up to $25,000 per day for each violation. The spill log should include how any spills were cleaned up and how the spill materials were disposed. The sample apparatus at bothsample points have issues with sediment buildup and needs to be addressed in a timely manner. A follow up report regarding the Zinc contamination and sample point issue investigation should be submitted to this office within 90 days. Include an outline of possible or implemented solutions to these issue.
