The comments from the referenced letter have been reproduced below and addressed as follows: 1. Please use the proper application form (SWU-101). The application can be found at the following link: Response: Using Application Form SWU-101 version Oct 31, 2013 downloaded from your link. 2. Please specify the type of project (Low Density, High Density, Off -site, or Other) in the application (Section II, 2). Please refer to the NCDEQ Stormwater Design Manual for help determining what type of development this project qualifies as. The NCDEQ Stormwater Design Manual can be found at the following link: Response: Project is low density based on impervious areas of less than 24%. 3. Please provide information on the local government that will be issuing the building permit (Section III, 4). This will assist us in getting a copy of the permit to the correct Building Inspection Office. Response: Town of Midland is the Local Government, contact information has been added to the application. 4. Please include the total area of surface waters on the project site (Section IV, 6). If there are surface waters present on the property site and the total surface water area is less than0.01 acres, you may write "< 0.01" in this section and treat it as zero for calculation purposes. Response: Wetland areas have been identified for this parcel but are not in the proposed development areas. Area is included on this application (Section IV, 6) 5. Please include a signed, sealed, and dated Supplement-EZ Form for the proposed BMP (Section V). The form can be found at the following link: Response: Supplement --EZ has been completed. 6. Please enter the relevant drainage area information into the tables under Section IV, 10. Response: Drainage area information has been added to Section IV, 10 7. Please identify any wetlands that are located on the project site. If no wetlands are present, please provide a note on the plans stating so. Please also provide the name and contact information for the person who made this determination (Section VI, 8, m). Response: Deli►ieation of wetlands by CWS is attached with this submittal 8. Please provide a plan sheet showing the entire drainage area for the proposed BMP (Section VI, 8, o). Do not forget to include the facility itself in this delineation. Response: Plan sheet showing drainage area to the BMP is included with this submittal. Please provide a soils report based on an actual field visit. This report should contain the Seasonal High -Water Table, SHWT (Section VI, 9). This is an MDC for sand filters. Response: Soils report is attached. Ground water was not found 10. The storage volume in the sediment chamber and the sand chamber of the sand filter should be equivalent (Sand Filter MDC 2). Response: storage volumes are now equivalent 11. Provide riprap sizing calculations to show that flows into the BMP and out of the BMP are non -erosive (General MDC 4). Response: Riprap calculations are included with this submittal 12. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other documents. Pdfs must be uploaded using the form at: Response: Submittal copies of all changes have been sent with this letter. While it is not required, it is recommended that: - Calculations showing that water flowing from FES-2 around the proposed BMP will not travel at an erosive velocity or will not overtop the embankment and flood into the sand filter. - The top of grate elevation and top of emergency spillway weir elevation are shown as different values on the Sand Filter Detail and Outlet Structure Detail on sheet C-404. Response: Discrepancy has been corrected. - A stage storage table be included on C-404. Response: Stage Storage Table has been added.
