Dear Mr. Farkas,
In response to your comments of November 8, 2021, please find the following revised
documents:
• Sheets C14.1, C14.2, C15 & C18.
• Original signed (in pen) & notarized hard copy of the latest version of the
Operation & Maintenance Agreement Form
The following changes were made:
1x original signed (in pen) & notarized hard copy of the latest version of the
Operation & Maintenance Agreement Form. This item is required per Section V and
Section VI, 3 of the Application and the form can be found at the following address:
https:Hdeg.nc.gov/about/divisions/energy-mineral-and-land-
resources/stormwater/stormwater-program/stormwater-design. The latest version of
this form has a "low -density" option that should be used instead of the "grassed
swale" option. NOTE: You do not need to include the dry ponds in the O&M
Agreement Form (since they are not required for a low -density project), but if you
would like to include them in the O&M Agreement Form (to ensure that they get
properly maintained), place a note on the cover sheet of the O&M Agreement Form
indicating that these dry ponds are not required per this permit. NOTE: You may
wish to get a second original, signed & notarized hard copy of this form to keep with
the permit since an original copy is required to be recorded with the Register of
Deeds.
New Operation & Maintenance Agreement is submitted.
2. 2x signed & sealed hard copies of the plan sheets showing the following:
o Drainage areas to the proposed vegetated conveyances (Required per
Section IV, 8o of the Application & 15A NCAC 02H .1042(2)(g)(iv))
Sheet C14.1 was added to show drainage areas.

o Delineated wetlands or a note saying that no wetlands are present on -site.
The name of the individual that made this determination must also be
included on the plans (Required per Section IV, 8m of the Application &
15A NCAC 02H .1042(2)(g)(iii)). It is noted that the cover sheet contains a
note stating that the wetlands are "as shown on plans", however I did not
notice any delineations or note on the plans saying that the wetlands are
not present.
Wetlands (W1-W12) are located in the NW of the site. Report from
Hal Owen & Associates dated February 9, 2021 is included for your
reference.
3. There are 3 additional swales (14-16) that were not labeled in the plans
previously sent. Revisions were made to sheets C14.2 and C15. Attached
are the calculations for those swales.
4. Sheet C18 was modified to update the O&M's for Low Density and Dry
Ponds.
